1. Introduction
These Terms of Sale (“Terms”) govern all transactions, purchases, and services provided by Guardian Health Agency. By purchasing a policy, enrolling in a plan, or using our services, you agree to these Terms. If you do not agree, please do not use our services.
2. Services Provided
Guardian Health Agency is a licensed health insurance brokerage that helps individuals and families explore, compare, and enroll in Affordable Care Act (ACA) health plans and other insurance products. We act as an intermediary between clients and insurance carriers but do not underwrite or issue insurance policies ourselves.
3. Pricing and Payments
Quotes provided through Guardian Health Agency are based on the information you submit and are subject to verification by the insurance carrier.
Premiums, fees, and payment terms are determined by the insurance carrier, not Guardian Health Agency.
Some services may be free to you as we are compensated directly by carriers. Where applicable, any fees will be disclosed in writing before a transaction is completed.
You are responsible for paying premiums and any related charges directly to the insurance carrier unless otherwise stated.
4. Enrollment and Transactions
By submitting an application through Guardian Health Agency, you authorize us to assist in transmitting your information to the appropriate insurance marketplace or carrier.
Enrollment is not guaranteed until confirmed by the insurance carrier.
We are not responsible for delays or rejections caused by incomplete, inaccurate, or fraudulent information provided during enrollment.
5. Refunds and Cancellations
Guardian Health Agency does not directly process premium payments; therefore, any requests for refunds or cancellations must be made through the insurance carrier.
We will assist you in contacting the carrier if you wish to cancel or adjust your coverage.
Service fees (if applicable) are non-refundable once services have been provided.
6. Client Responsibilities
You agree to:
Provide accurate, current, and complete information during the enrollment process.
Review your application and policy documents for accuracy before submission.
Notify us promptly of any changes to your contact, household, or income information that may affect your eligibility or coverage.
7. Limitation of Liability
Guardian Health Agency is not responsible for decisions made by insurance carriers, including claim denials, premium changes, or coverage determinations.
We are not liable for indirect, incidental, or consequential damages arising from your use of our services.
Our role is advisory and administrative; the final terms of coverage are between you and the insurance carrier.
8. Changes to Terms
Guardian Health Agency may update these Terms of Sale at any time to reflect changes in our services or applicable laws. Updates will be posted on this page with a revised “Last Updated” date.
